Job Description

SUMMARY:  A Dallas Hospital system is seeking an experienced coding specialist to join their team for a 6 month project with the option to be extended.

RESPONSIBILITIES: 

1. Responsible for coding retrospective Observation (OBS) Medical and Surgery accounts using ICD-10 CM and CPT, in compliance with the Official Coding Guidelines and conventions. 
2. Accurately identifies most appropriate Reason for Visit, First Listed (Primary) Diagnosis, Procedure(s), Modifiers, and all secondary diagnoses to accurately support medical necessity (LCD/NCD edits) and CCI edits. 
3. Accurately abstracts required data elements including, discharge disposition, discharge destination, procedure dates and physician(s), Occurrence Codes, and other designated data.
4. 
a) Identifies when a physician query is appropriate for further clarification. 
b) Recognizes when the documentation is missing or incomplete and routes appropriately. 
c) Validates chargemaster driven CPT code assignment and routes appropriately. 
d) Completes review and final coding when query and/or documentation is available. 
5. Maintain the Quality Standards set by HIM Coding. 
6. Maintain the Productivity Standards set by HIM Coding. 

REQUIREMENTS: 

Preferred Bachelor's Degree or Associate Degree in Health Information Management and/or closely related field 

REQUIRED CERTIFICATIONS/LICENSE 
Registered Health Information Administrator (RHIA),

Registered Health Information Technician (RHIT),

Certified Coding Specialist (CCS),

Certified Outpatient Coder (COC),

Certified Professional Coder (CPC),

Certified Inpatient Coder (CIC),

Certified Professional Coder-Hospital (CPC-H),

or Certified Interventional Radiology Cardiovascular Coder (CIRCC) 
 

ADDITIONAL INFORMATION 
-Coders will be required to clock time via Kronos 
-Kronos auto-deducts 30 minute lunch after 7 hours and during 8 hour work day 
-Vendor must provide a trainer/auditor for their employees 
-100% coding review, until meeting 95% quality 
-Expectation is to be meeting quality and productivity standards within an established time period as determined by the Client 
-Productivity Standard – Determined by the Client who will provide productivity standards during onboarding process 
-Must provide quality reports quarterly for employees working in the previous quarter 
Ex: employee worked Jan, Feb, Mar, quality report due by the 15th of April 
-Additional auditing may be requested by UTSW if quality issues are identified by UTSW during the pre-bill review process 
-Coders are required to complete all requested online compliance training upon accessing the Client’s computer, within Client’s mandated timeframe 
-Compliance Training is billable up to a maximum of seven hours at the pay rate specified by the Client 
-Coders are required to meet any additional quality and training standards established by the Client 
-Teaching Hospital Experience preferred 
-Required for vendor to provide coders with computer or external hard drive/remote desktop 
-High Speed Internet required 
-Dual monitors required – laptop not acceptable 
-Required for vendor to provide coders with Skype for Business 
-While working, coder must log in to Microsoft Outlook, and only use corporate email to communicate account information 
-Coders must have an ability to apply Official Coding Guidelines, and follow guidance provided in CPT Assistant, Coders' Desk Reference, Coding Clinic for ICD10CM/PCS, and Coding Clinic for HCPCS to validate code assignment 
-Coders must apply appropriate/applicable modifiers 
-Coders must review medical necessity and CCI claim edits 3 times weekly in claim edit WQ 
-OBS Coding Productivity Standard 3 Cases/Hour 
-Day Surgery Coding Productivity Standard 5 Cases/Hour 
-Emergency Department Coding Productivity Standard 10 Cases/Hour
